What the Form F Assessment Covers
The Form F assessment is a written assessment that will explore your life in full, not just your current circumstances. It will seek information about your childhood, your previous relationships, your past and present employment, your parenting experiences and your ability to manage children’s behaviour. As much as we would like the assessor to have a nice easy read of your history, it is more important that they have an insight into your self awareness and how you are in real life.
How the Matching Panel Works
The Matching Panel consists of an independent chairperson, social workers and in some cases representatives from voluntary agencies and therefore you will have the opportunity to present your circumstances to them. They will ask you questions regarding your ability to manage children’s behaviour including scenarios such as dealing with a child who is violent or refuses to engage with you.
Home Environment Checks
Home environment: Assessors will visit your home and look for signs of safety i.e. fire in all rooms, safe storage of medicines, sufficient private space for a child and other indicators of safety. Make sure any obvious hazards are fixed before the visit. Also, make sure that all garden gates and fences are secure.
References, DBS, and Medicals
References (and these can be personal), a full DBS check, a medical from your GP and any employer references needed (and these should be requested as soon as your assessment begins and can take several weeks for the GP’s medical).
Approval Conditions
In Wales, Foster care approval is regulated in Wales by how foster care approval is regulated in Wales. This means that the standards for how Foster Care approval is regulated in Wales are applied by Foster Care approval panels in Wales. This means that your approval may be conditional upon approval to look after children of a certain age, or even a certain number. Your assessing social worker will be able to explain any such conditions upon your approval in full.
